Gippsland PHN is seeking Expressions of Interest (EOI) from eligible East Gippsland-based small or solo General Practices and Aboriginal Community Controlled Organisations (ACCOs). The EOI is to establish and deliver a multidisciplinary team care model for patients with, or at risk of, heart failure.
Total funding of $441,221.85 (excluding GST) is available for program delivery from 30 November 2026 to 30 June 2028.
